Advocacy Form We need your help to illustrate the personal impact of diabetes on millions of Americans by bringing real faces and stories of children with diabetes to the attention of Washington lawmakers. Please complete the form below with a letter from your child to your Member of Congress relating his or her story of what it’s like to live with diabetes and why a cure is needed. Family and friends may also write in support of JDF Children’s Congress and the need for increased funding for research to find a cure. Please also send an e-mail message to childrenscongress@yahoo.com with a picture of your child as an attachment (pict, tiff, jpeg, or gif format). You may also send letters directly via e-mail; however, please send plain-text or rich-text formats only, as it is sometimes not possible to convert proprietary word-processor files.
