[Dataloss] DLDOS Update: New "Type" Classification

In order to distinguish between certain types of data loss events, the 
"Type" column now has a new subset:  "Disposal".

Previously, data lost through improper disposal was classified as either 
"Lost Media" or "Fraud - SE" depending on circumstances.  Recently, 
several events have been found (either recently or in years past) as 
having been the result of improper disposal of personal information. 
Since the media seems to have jumped on the "dumpster dive" fad in the 
last few months, we'll try to distinguish between "disposal" and truely 
"lost media".
